Bridges to Health: Programs & Pathways
Bridges to Health (B2H) brings together diverse community and university partners to develop and implement sustainable, trauma-responsive programs that aim to reduce health disparities and expand access to health education. By addressing both clinical and social determinants of health, B2H fosters pathways to wellness and aims to promote community health and wellbeing through collaboration with Federally Qualified Health Centers (FQHCs), schools, faith-based organizations, pharmacies, 51°µÍø and philanthropic organizations.
DUCIH’s programs and initiatives are designed to promote health and wellbeing through community-centered approaches. Our work falls into six key focus areas:
- Social Determinants of Health

- Chronic Disease Prevention and Management
- Maternal Child Health
- Mental Health and Wellbeing
- Integrative Approaches to Health
The B2H Faculty Fellowship Program fosters cross-disciplinary engagement, bringing faculty and students together to lead sustainable, innovative projects that aim to strengthen community health and advance the Center’s mission.
Examples of B2H Health Equity Faculty Fellow project topics:
- Substance use education program for middle school students and families.
- Promoting mental wellbeing and spirituality in older adults.
- Supporting the wellbeing of caregivers for infants and young children.
- Advance directive planning and assistance for community members.
- Maternal-child health training program for nursing students.
